Time and Place of the Study The study is currently conducted from July 2012 up to present at Patrico-Valle Maternity and Surgical Clinic for the interview and for the document and system; it was conducted in Cavite State University Imus-Campus. Scope and Limitations The system runs in computer that uses Windows XP Service Pack 1, 2, 3, Windows Vista and 7 for operating system. Other requirements are MySQL 5. 5 and Visual Basic 6. 0, which is incorporated upon installation of the system. MySQL 5. 5 will serve as the backend or database of the file system. This system can add, eliminate and renew the information of the patient. If the information of the patient change, it will be retrieve right away and easy to update. No need to write again the information. It can also recover the history of the patient as well as the billing records of the patient. The system limits if the patient purchased medicine. Only the rendered service like operations and check ups are billed. Only the accountant and the person assigned to get the patient’s information can operate and use the system.
